When the activities of daily living become an effort or unsafe for frail elders, the chronically ill or the physically disabled, Oast & Hook’s care manager is ready to support these endangered individuals in the least restrictive setting of their choice. It is the goal of the care manager to enlist the best mix of community, family and personal resources, and to work closely with lawyers, physicians, accountants, financial planners, health care and long term care agencies to create a plan for the individual and their family.

In every instance we strive to provide assistance to senior citizens or disabled persons in a professional manner while maintaining the dignity of our clients and their families. We act as advocates, maintaining continuity of care to prevent unnecessary hospitalization and costly crises. As a liaison between the family and health-care community, we offer security and 24-hour availability to reduce stress and costs to our clients.

On a daily level, our care manager may pay bills for the client who can no longer manage a checkbook, or go to the emergency room in the middle of the night to make sure the hospital has all current and correct information on the patient. Being alone in an emergency room can be a frightening experience for anyone, but for someone with a memory deficit it could prove catastrophic. Some elders are without family or live far from their relatives.  At times like these, our care manager is the familiar face restoring calm, keeping in contact with distant relatives, offering reassurance and comfort. Our care manager becomes the client's advocate, and contact to the outside world.

As an Elder Law firm, Oast & Hook provides much more than legal and financial service. Our care giver can arrange for groceries to be purchased and delivered, yard work to done, caregivers or companions. We begin with an estate plan prepared by attorneys that is researched and reviewed by our in house accountant and trust officer. Then we create continuity within a team framework coordinating community and family resources as necessary and monitoring those services to ensure a consistent level of care.
