When the activities of daily living become an effort or unsafe for frail elders, the chronically ill or the physically disabled, Oast & Hook’s care manager is ready to support these endangered individuals in the least restrictive setting of their choice. It is the goal of the care manager to enlist the best mix of community, family and personal resources, and to work closely with lawyers, physicians, accountants, financial planners, health care and long term care agencies to create a plan for the individual and their family. In every instance we strive to provide assistance to senior citizens or disabled persons in a professional manner while maintaining the dignity of our clients and their families. We act as advocates, maintaining continuity of care to prevent unnecessary hospitalization and costly crises. As a liaison between the family and health-care community, we offer security and 24-hour availability to reduce stress and costs to our clients. On a daily level, our care manager may pay bills for the client who can no longer manage a checkbook, or go to the emergency room in the middle of the night to make sure the hospital has all current and correct information on the patient. Being alone in an emergency room can be a frightening experience for anyone, but for someone with a memory deficit it could prove catastrophic.
